Triage and Care Coordination Nurse (Remote Registered Nurse)

The Triage and Care Coordination Nurse (TCCN) is a remote registered nurse that provides telephone and electronic triage support to firsthand patients and staff, and supports outpatient care coordination. They work 8 hour shifts, currently working hours only, but with a likelihood to include evenings and weekends in 2024. TCCNs are expected to be doing care coordination, training and related administrative work when not managing acute issues. TCCNs will be 1099 contractors contracted on an hourly basis.

Michigan RN license and NLC compact license are required.

As a Triage and Care Coordination Nurse you will:
  • 16-24 hours a week on a remote basis, paid at an hourly rate of $40
  • Working hours shifts will be 8 hours (9-5 EST/8-4 CST)
  • Receive inbound clinical issues from firsthand staff and patients directly by phone and Google forms and triage appropriately including escalating true urgencies/emergencies immediately
  • Collaborate closely with firsthand staff, including peer mental health workers, social workers, and APPs to address acute issues as early and comprehensively as possible
  • Coordinate care with our patients’ other care providers
  • Develop and deliver training on basic medical issues to peer mental health workers and social workers

The experience you bring to this role includes:
  • You must be a Registered Nurse with active RN license through a Nurse Licensure Compact (NLC) state and willing to get licensed in other non-compact states
  • 5+ years clinical care experience including triage in Emergency Department and/or Primary Care settings
  • Working with and managing a population with challenges such as behavioral health and/or SUD conditions
  • Care management and coordination

$40 - $40 an hour

This is a part-time, non-exempt, hourly position reporting to firsthand's Director, Clinical Operations . firsthand cares deeply about ensuring pay equity with every team member who joins. Commensurate on candidate experience, the expected hourly rate is $40/hr.
